Utah Driver Jailed After 3 Disabled Clients Die of Suspected Carbon Monoxide

State health officials imposed a 30-day restriction on the provider pending multiple investigations.

Overview

  • Police say the driver left three men in a running vehicle inside his garage around 10 a.m. and returned roughly 3.5 hours later to find them unresponsive from apparent carbon monoxide poisoning.
  • The driver, identified in arrest documents as 25-year-old Isaiah Pulu, was booked into the Salt Lake County Jail on suspicion of three counts of manslaughter and two counts of aggravated abuse of a vulnerable adult.
  • Utah’s Department of Health and Human Services issued an emergency notice barring Safe and Sound Services from taking new clients for 30 days, mandating immediate staff retraining, increased monitoring and a licensing investigation.
  • Officials said the provider has until Wednesday to request an administrative hearing, and as of Monday evening no request had been filed.
  • The victims—Colton Moser, 25, Mosa’ati Moa, 22, and Tim Jones, 39—lived in separate group homes, the Disability Law Center launched an independent review, the company’s website went offline, and families began raising funds for funerals.
