RAIB Faults Elizabeth Line Safety After Passenger Dragged, Issues Five Recommendations

The investigation sets out fixes for operators to cut trap-and-drag risk across the network.

  • RAIB’s report details a November 24, 2024 incident at Ealing Broadway where a westbound train departed with a passenger’s hand trapped, dragging them about 12 metres before they were pulled free.
  • Investigators concluded the driver closed doors while people were still boarding or alighting, the door system failed to detect the trapped hand, and the driver was unaware before initiating departure.
  • Ineffective station risk controls under then-operator MTR Elizabeth line were cited, with a possible shortfall in Network Rail’s risk assessment after a waiting-room relocation.
  • Recommendations call for GTS Rail Operations to strengthen control of trap-and-drag hazards, for TfL to improve driver-only CCTV views and evaluate technology, for RSSB to update standards, and for Network Rail to better assess safety impacts of infrastructure changes.
  • TfL says new signage and platform markings are in place and it is working with GTS and Network Rail on the actions; RAIB also flagged weak safety communications, poor PA connectivity, and gaps in DOO CCTV testing standards.
