Oldham Review Finds Systemic Failures After Boy, 10, Treated for Scurvy Under Care Order

A newly published review details multi-agency failures, prompting an apology from Oldham’s safeguarding leaders.

Overview

  • The safeguarding review released in November 2025 says school staff filed 38 alerts about rapid weight loss and lunchboxes containing dry or mouldy bread with water, but concerns were not escalated.
  • Social services were moving to step down his care plan as his condition worsened, while two A&E visits and a GP appointment failed to identify malnutrition and scurvy.
  • A school nurse secured his hospital admission, where he was diagnosed with malnourishment, anaemia and scurvy and had four teeth removed.
  • The report notes he struggled to walk from vitamin deficiencies, was made to walk back from a medical appointment, and later flagged down a stranger for a lift.
  • Oldham Safeguarding Children Partnership issued an apology through councillor Shaid Mushtaq and says practice changes are in place, while the full care order at home remains.
