NTSB Finds Trench Faults and Call Failures Caused Deadly Maryland House Explosion

The July 15, 2026 NTSB report attributes the blast to an overheated tracer wire that melted a gas pipe, citing caller-routing failures and lapses in fitness-for-duty.
