NTSB Blames Chain of Failures for Deadly Bel Air House Explosion

The report says technical faults, missed dispatches and an unresponsive call agent allowed gas to collect then ignite, prompting utility changes.

  • The National Transportation Safety Board released its report on July 15, 2026, saying a sequence of technical, communication and human-factor failures led to the August 11, 2024, Bel Air explosion that killed a homeowner and a BGE contractor.
  • Investigators found an underground electrical fault energized a copper tracer wire that heated and breached a nearby plastic gas service pipe, allowing gas to leak and migrate to the house; the report noted gas and electric lines in the trench were as close as three inches, far below the 12-inch recommended separation.
  • Multiple reporting and dispatch failures delayed a proper response for more than 10 hours, including a misrecorded location that sent a gas crew to the wrong address and automated routing that sent emergency calls to an on-call agent who was unresponsive during an unscheduled overtime shift.
  • The NTSB said Exelon did not submit the on-duty agent for post-accident drug and alcohol testing, so investigators could not quantify the agent’s impairment, and the missed calls left repeated reports of gas from neighbors and a county worker unaddressed before the blast.
  • BGE and parent Exelon have adopted fixes the report described, such as forcing manual answering on overnight and weekend shifts, supervisor fitness screenings for call-takers, new technician protocols at shared-trench sites, and a pilot program for physical trench barriers to reduce the risk of pipe damage.
