Overview
- Dr Ajith Gurusinghe, speaking Monday, acknowledged he did not record Valdo Calocane bringing a hammer onto The Priory ward in October 2021 and said the tool was stored then returned at discharge.
- Helen Foster, a charge nurse at the Priory, told the panel Monday that the hammer episode was not followed up and said poor incident logging meant key risks were hidden from staff.
- GP Tim Baker testified Tuesday that Calocane should not have been discharged to primary care after disengaging from specialist services and said his severe mental illness then went untreated.
- Consultant psychiatrist Jonathan Gibson warned Tuesday that cases like Calocane’s could become more common because there are fewer beds, sicker patients and a push for the least restrictive care.
- The hearings continue in London with evidence from Calocane’s family scheduled this week and the chair due to report findings and recommendations by May 2027.
