Norfolk Review Finds Fundamental Safeguarding Failures in Pensioner’s Murder by Lodger

Officials pledge new training with in-person checks following a report that documented ignored warnings.

Overview

  • The Norfolk County Community Safety Partnership published a domestic homicide review on 9 September detailing multi-agency failures to recognise the risk Allan Scott posed to 83-year-old Patricia Holland.
  • The review says Holland and her family sought help at least 20 times to have Scott removed, yet agencies did not share information or coordinate an effective safeguarding response.
  • Missed measures included a home visit, a mental capacity assessment, consideration of non-molestation or exclusion orders, and proper scrutiny of Scott’s criminal history.
  • The report makes 34 recommendations, urging greater weight to family concerns, better recognition of “cuckooing,” and complete background information in safeguarding records.
  • On 10 September, Norfolk County Council accepted it should have done more and said changes are under way, while police cited new domestic abuse training, updated information-sharing in the MASH, and IOPC findings of no misconduct.
