Hospital Trust Says Safety Changes Made After Coroner Finds Missed MRI Delay in Pregnant Mother’s Death

An inquest found Tammy Mackenzie’s death was natural but contributed to missed chances to diagnose a small bowel obstruction earlier.

  • The internal post-incident review told the inquest that an earlier MRI would likely have led to earlier treatment and increased the chances of survival for both mother and baby.
