Overview
- HHS published a Request for Information on Aug. 24, 2026 asking whether the current three-category framework for vaccine guidance should be revised and offering sample labels such as “recommended with qualification” and “recommended, but not during infancy.”
- The agency invites comment on evidence standards, how to make decisions when randomized trials are unavailable, the balance between individual versus population benefit, and a presumption in favor of individual autonomy and religious freedom with comments due by Sept. 20.
- The RFI raises the prospect of aligning U.S. guidance with foreign peer agencies and explicitly asks respondents to describe the intended downstream effects of any new category on coverage and implementation.
- Medical groups and legal advocates have criticized the move as part of a broader White House effort on vaccine policy, and a federal judge earlier this year temporarily blocked many of HHS Secretary Robert F. Kennedy Jr.'s prior recommendation changes and ACIP appointments.
