DoD Watchdog Faults Ex-Ukraine Support Commander for Lost Secret Maps and Alcohol-Related Concussion

The Pentagon inspector general’s March 12 report calls for a security review with potential Army action after confirming policy violations.

Overview

  • Maj. Gen. Antonio Aguto left Secret-classified maps on a State Department–chartered train on April 4, 2024, losing control of them for about 24 hours before recovery via the U.S. Embassy in Ukraine.
  • The investigation found the maps were improperly stored in a tube and not moved by authorized courier, violating a 2022 Chief of Mission directive governing cross-border transport of classified materials.
  • The inspector general recommended referring the mishandling to the U.S. European Command Special Security Office and said the Army secretary should consider appropriate action.
  • Aguto overindulged in alcohol during a May 13, 2024 dinner in Kyiv, suffered multiple falls, and was diagnosed with a moderate-to-severe concussion that affected his performance the next day.
  • The report did not substantiate claims of a toxic command climate; Aguto accepted responsibility for the map incident and had already relinquished command and retired in August 2024.
