Coroner Says DWP PIP Error and Letter Contributed to Woman's Death, Response Due by March 19

A prevention-of-future-deaths report has been sent to Work and Pensions Secretary Pat McFadden requiring a formal reply.

Overview

  • Senior coroner Alison Mutch found the removal of Tamara Jade Logan’s enhanced daily living PIP award was an incorrect determination and said the standard letter used to notify her was inappropriate given documented vulnerabilities.
  • The inquest concluded her death was suicide, with the medical cause recorded as hypoxic brain injury resulting from hanging.
  • Evidence showed DWP records flagged prior self-harm and suicidal ideation, the initial assessment was carried out correctly, and a subsequent checking stage failed to catch the error.
  • The prevention-of-future-deaths report has been issued to the Department for Work and Pensions, with a statutory deadline of 19 March 2026 for an official response.
  • The DWP offered condolences, said it will provide a full response, and pointed to safeguarding measures such as Vulnerable Customer Champions, CEAST, ACSSLs, a multi‑disciplinary support team, and wider reforms under the Timms Review.
