Overview
- An April 2026 inquest recorded a conclusion of suicide for Pawel Bukowski, with the coroner saying his mental-health crisis followed failed dental work in Turkey and citing messages and notes as evidence of intent.
- Mr Bukowski, who had periodontal disease, travelled in January 2025 for implants, but the clinic removed all his teeth, then sent him home with no temporary dentures and told him to return in six months.
- He struggled to eat and began drinking heavily as his mood worsened, and emergency services took him to Norfolk and Norwich Hospital on April 24, where a liaison practitioner judged him at high suicide risk but he was not admitted.
- He was referred to a crisis team and given a small supply of medication, yet a nurse withheld more due to heavy drinking, and a psychiatrist who arrived three hours late on April 28 found him hanging at his home in Watton.
- An NHS trust review identified lessons and a possible missed opportunity to admit him, while his wife said workers missed appointments and calls and that he received no help with alcohol withdrawal.
