Overview
- The government launched the PM Family Care Tracker pilot on Sunday, June 28, in the Gandhinagar Lok Sabha constituency and issued the first digital Health Passports while conducting a pulse polio drive and distributing nutrition and Anganwadi kits.
- The platform consolidates existing systems by using the ABHA digital health ID and the Birth Registration Number to link TeCHO+, Poshan Tracker, the Child Tracking System and other departmental databases.
- About 16 central and state schemes covering maternal care, immunisation, nutrition, education and social welfare have been integrated so records and benefits can be matched on a single portal.
- PM-FCT generates automated alerts for missed vaccinations, underweight or at-risk children, and school dropouts and escalates cases to taluka and district officials as well as elected representatives for follow-up.
- Officials present the pilot as a step in the post-2014 digital and Direct Benefit Transfer push to reduce omission and improve last-mile delivery, and they will evaluate the Gandhinagar pilot before expanding it across Gujarat or to other states while an age-coverage detail (16 versus 18 years) remains inconsistent in public accounts.
